Complications and Complication Management in Oral Surgery and Implant Dentistry
Complications and Management of Complications, Retreatment of Failures in Implant Dentistry and Oral Surgery
Complications and their retreatment are frequent occurrences in dental practice and knowledge of the management of such cases is essential.
This lecture will provide the dentist with surgical and implant protocols with a realistic, honest and practical overview of possible complications in dental surgery and implantology, how to overcome them with successful and predictable management outcomes, and retreatment that may be necessary and indicated.
Case documentation and treatment options will be used to illustrate complication management strategies.
The presentation is designed as a brief guide to the diagnosis, prevention and management of complications in dental surgery and implant dentistry. It will demonstrate how to prevent errors in surgical and implant procedures and how to manage complications.
The goal of the lecture is to understand the mechanics of these errors and to enable the clinician to formulate a more predictable treatment plan and to reduce the fear of possible complications.
Preoperative Complications
Intraoperative complications of implant placement
Implant Complications
- Flap design
- Incorrect implant angulation
- Deep/shallow implant placement
- Injury to adjacent teeth
- Nerve injury
- Bleeding
- Bone overheating during drilling
- Excessive torque during insertion and compression necrosis
- Perforation of the sinus floor
- Accidental partial or complete dislodgement of dental implants into the maxillary sinus
- Accidental dislodgement of dental implants into the maxillary incisive canal or alveolar nerve canal
nerve canal
- Aspiration or ingestion of foreign material
- Implant fracture
- Mandibular bone fracture
- Insufficient initial stability
- Bleeding during incision and flap reflection
- Perforation of sinus membrane during elevation
- Incomplete elevation
- Bleeding during elevation
- Excessive elevation of the membrane
- Presence of mucous retention cyst
- Blockage of the maxillary ostium
- Unstable implants
Postoperative complications
- Postoperative pain
- Tissue emphysema induced by dental procedures
- Cover screw exposure during healing
- Bone growth over the cover screw
Complications associated with lateral window sinus elevation
- Acute graft infection/sinusitis
- Chronic infection/sinusitis
- All sinus/intracranial infection
- Delayed migration of the implant into the sinus cavity
